CHILD DEDICATION REQUEST FORM Children are the Reward of Life! (African Proverb) |

| CEREMONY: Friendship-West Baptist Church (FWBC) though DISCOVER, FWBC’s Children’s Community dedicates children under two years old during the 10:45 a.m. worship service on 3rd Sundays. Private dedication ceremonies may also be requested. STEPS: Request forms are due the 3rd Sunday at least one month before the desired dedication month. Please submit completed forms in the Connection Center mailbox (south Narthex) or online. NOTES: Sunday dedication slots are limited. Dedication dates are assigned 1st come, 1st served. ALL dedication parents must participate in a 75 minute session. Parents unmarried when their child was conceived will also participate in a 55 minute session. You will receive an invitation to these sessions which are usually on the 1st Mondays. Saturday sessions are available quarterly. Please call 972-228-5200 for additional information. |
